    Full name of map and version


    Type of map


    Reason on why do you want it removed from the server

    Instructions:

    1) Register in the forums. Only members can post in forums.
    2) Create a new topic.
    3) The post title of the topic you have created should be like this: Map Removal - "full name of map"
    4) Fill in the form like the post above with your own answers.
    5) Add a poll. Poll's title should be "Remove?" with the options of only "Yes" and "No".

    You can refer to the example in the first post.
